What Is The Rotator Cuff?

The rotator cuff is a group of four muscles that stabilise and control movement of the shoulder joint. These muscles help lift, rotate, and support the arm during daily and sporting activities.

The rotator cuff plays an important role in:

  • Shoulder stability

  • Overhead movement

  • Lifting and carrying

  • Throwing and sporting activities

  • Controlling shoulder mechanics​

When the tendons become overloaded, irritated, or torn, shoulder pain and weakness can develop.

What Is Rotator Cuff Tendinopathy?

Rotator cuff tendinopathy refers to irritation, overload, or degeneration of the rotator cuff tendons. The tendons are the connection between the rotator cuff muscle bodies and your bones, they are made of a connective tissue which helps to store and transmit force created by the muscles. A tendinopathy commonly develops gradually over time due to repetitive strain, poor shoulder mechanics, or an increase to activity levels.

Common Symptoms Of Rotator Cuff Tendinopathy

  • Pain lifting the arm

  • Pain reaching overhead

  • Shoulder weakness

  • Pain sleeping on the affected side

  • Discomfort during gym or sporting activities

  • Stiffness or aching around the shoulder

Rotator cuff tendinopathy often responds very well to physiotherapy and progressive strengthening programs.

 

What Is A Rotator Cuff Tear?

A rotator cuff tear occurs when one or more rotator cuff tendons become partially or completely torn. Tears may occur suddenly after trauma or develop gradually over time through tendon degeneration and chronic overload.

Common Symptoms Of A Rotator Cuff Tear

  • Sudden shoulder pain after lifting or injury

  • Significant weakness

  • Difficulty lifting the arm

  • Pain during overhead movement

  • Clicking or catching sensations

  • Ongoing shoulder pain and reduced function

Some tears may be managed successfully with physiotherapy, while larger or traumatic tears may require specialist review and/or surgery. 

 

 

How Do Rotator Cuff Injuries Occur?

Rotator cuff injuries commonly occur from repetitive overload, poor shoulder mechanics, or sudden trauma.

Repetitive Overhead Activity

Repeated overhead movements during work, gym training, swimming, tennis, or throwing sports can overload the rotator cuff tendons.

Heavy Lifting

​

Lifting heavy objects, especially away from the body, or in overhead positions may strain or tear the rotator cuff.

Falls Or Trauma

​

Landing on an outstretched arm or sudden shoulder trauma can cause acute rotator cuff tears.

Age-Related Degeneration

​

Rotator cuff tendons naturally undergo changes over time, making tears more common with increasing age.

This begins with a detailed assessment of your injury, including a conversation about your relevant history, any concerns or questions you have about the injury and identifying goals that you have in regards to the activities that you want to get back to.

Progressive exercise is a crucial element of your rehabilitation, it’s important for optimal healing and results. Overloading the shoulder with excessive exercise can make it feel worse, and underloading it with exercises too easy can often feel like there is no change in the shoulder. That's why we use our assessment to correctly prescribe appropriate exercises to you at every stage of your rehabilitation.

Frequently Asked Questions

Do all rotator cuff tears require surgery?

​

No. Many partial tears and some full-thickness tears can improve significantly with physiotherapy rehabilitation and strengthening. Factors such as age, activity levels and size/location of the tear help to guide this decision. 

​

How long does rotator cuff rehabilitation take?

​

Recovery varies depending on the severity of the injury. Mild tendinopathy may improve within weeks, while more significant tears or tendinopathy can require several months of rehabilitation.

​

Can exercise make rotator cuff injuries worse?

​

Continuing to do aggravating activities such as throwing sports or excessive work overhead can exacerbate symptoms depending on the severity of the injury.  Appropriate exercise is however one of the most important parts of recovery. Exercises should be tailored and progressed carefully to avoid overload. 

​

Why does my shoulder hurt more at night?

​

Rotator cuff injuries commonly cause night pain due to tendon compression, inflammation, and shoulder positioning during sleep. Tendinopathy in particular has a tendency to hurt more at rest or in the time following activity when it has “cooled down”.

​

Can I still go to the gym or play sport with a rotator cuff injury?

​

In many cases, yes. All lower body exercise and sport will be unaffected, any exercises and training regimes involving the shoulder may need modification to avoid overload.  This is dependant on the severity of the injury.
